1. Valid VAT Registration Certificate/BIN. 2. Valid Income Tax Certificate/TIN, as applicable. 3. Recent Bank Solvency Certificate issued by a scheduled bank. 4. Line of Credit Certificate in prescribed PG3-8 format, where required. 5. Duly signed and notarized Affidavit/Declaration, where applicable. 6. Minimum 05 (five) years’ experience in supplying similar goods, supported by documentary evidence. 7. Valid Manufacturer’s Authorization Certificate, where applicable. 8. Citizenship Certificate and National Identity Card copy of the proprietor/authorized representative, where applicable. 9.
